  1. 1) The prophecy is based on perception. Most people consider(ed) Smith to be the eleventh Doctor, even he did. The correction of the numbering is only made to explain things to Clara. Since the Daleks recovered their knowledge of The Doctor from Tenzil Kem, she likely knew about the extra regenerations, so they knew he was at the end of his rope. But as far as the general public knew, it was the Eleventh that fell there.

Now, whether or not The Doctor has set up a paradox for himself is another question. He’s already been to Trenzalore a great deal of time in its future, and saw his grave, deep in the abandoned TARDIS. This is not hard to get around – there’s no reason not to assume that he returned to Tenzalore to finally die. The graves of centuries of fighting could easily have been there from the events of this latest episode, and his (and River’s fake one) were from many years later. Or perhaps not…maybe he came back mere moments later, relative to the eyes of the people of Christmas.

    2) The first attempt to return from the time lock in End of Time was by the Time Lords when they were still embroiled in the war, almost driven mad by the ravages of it. And by the time these events happened, that plan has long since failed as The general said in the anniversary special. It is assumed that for the Timelords, a great deal of time has passed within that bubble – time to atone, time to rebuild, and time to prepare. But the population of the universe, especially the Daleks, don’t know that, and don’t trust them.

When Gallifrey returns (if ever), it will have to be done in secret, far from prying eyes. I’m up in the air if they should come back, but that’s another story.
